This course addressed documentation issues that is inherent in the use of EMRs. While it is agreed that the use of these platforms can increasedocumentation efficiency there are major limitations. The presenter placed emphasis on the need for providing additional narrative documentationto provide a clearer more specific, less generalized, picture of the patient, his/her deficits, interventions provided and progress made. He had black and white ways to be better at documenting. Just write what skill the clinician or therapist brings to the client's deficit. Then document how the client responded. Track progress over time, Adjust therapy as patient progresses or fails to progress.

Critical review here: Disappointed in this presentation overall (and this is a rare occurrence). I'm appreciative of the why/who of the review process and the current situation as it provided good insight into the importance of getting documentation right the first time. However, I think this presentation somewhat speaks to the field of speech pathology being unappreciated and overlooked again. No examples at all of SLP documentation on a speech path platform. Interesting....

Though the documentation examples were of PT and OT notes and reports, the changes needed were very clear and can be easily applied to speech therapy documentation. I really appreciated that there were documentation examples as many of us really need examples to learn. The instructor provided a wealth of information overall.

Well organized. Concise. However, there is a gap between reality and "best practice". Exact ranges work well with adequate staffing, but often that is not the case. But when you have a slot open up you can actually see a resident for 5 days, usually not due to clinician time. Therefore the 3-5 days works well for the patient - homework on the days not seen. Nice information on the narrative vs. build of the documentation. I totally agree. Thanks for this informative session.

This video course provided an in-depth review of the world of EMR. The research data discussed the top reasons authorization for services may get denied. The criteria inclusion for appropriate goal writing and report writing was reviewed to provide comparison of appropriate vs inappropriate documentation. The implementation of EMR has improved efficiency however the incidence of being audited has increased due to the generalized documentation being utilized by healthcare providers.
