Good course. Thank you. I would have liked to see a few more swallows with each case and, at first, without commentary, then repeating those swallows with the commentary while pointing to the deficits. This would be a good way for me to test my skills reading the FEEs and then confirm that what I'm seeing, and why, is what you've seen. (I was right there with you for most of the interpretation but when you used the arrow, it just solidified it.). I would use the barium too. :)

I liked that she incorporated compensatory and treatment strategies specific to each case. I was wondering why she would proceed with thickened liquids if thin liquids were not aspirated? Penetration is normal in>65 y.o people. It would have been helpful to know more about Case #1's medical history and dysphagia etiology as it was not clear (no neuro diagnosis, intubation, etc mentioned).

The quality of the FEES videos was quite poor and often very pixelated which made it difficult to observe the anatomy and the movement of the bolus. Additionally, presenter did not provide adequate explanations for some questions (e.g. how to differentiate aspiration during vs. after the swallow during FEES).

I love the case studies and real videos that show what is happening. I also like that there is a debrief explaining. I would recommend a better debrief to summarize if possible. There are some times I think that the instructor is doing too much during the video and then too little in explaining what happened and what it means including recommendations.

This course would really have benefitted from frame by frame analysis of the swallows. It would make the physiological events much more apparent and enable learning clinicians to see what is actually happen. Additionally, it would set a good precedent for new-to-FEES clinicians about doing frame-by-frame review as that is essential.

Although some interesting tidbits of information were presented (e.g. Ingo Titze Straw Exercises for SOVT treatment), the pacing of the lecture felt rushed at times. For example, there was a push to show Case #4 because it was interesting but the learnings or case management from Case 3 felt rushed over in order to accomplish that. I felt like the course provided 4 case studies but didn't necessarily discuss advanced FEES interpretation, or key inferences that can be standardized or systematized for advanced SLP use.

For the untrained eye, it can be really difficult to see penetration/aspiration. Because the presenter has been doing FEEs for so long, this challenge to the novice seems lost on her. Skims over findings/imaging far too quickly without really honing in on how to clearly identify penetrated/aspirated material. A bit disjointed overall.

Key elements of the swallow cannot be accurately and consistently determined when viewed in real time. Freeze frame and slow motion videos are critical for determining precisely when and where the bolus crosses the laryngeal rim (in cases of penetration) or the plane of the vocal folds (in cases of aspiration). Slowing the video would also allow the clinician to identify the initiation of the swallow (via arytenoid and BOT movement) and correctly identify penetration and/or aspiration as occurring before or during the swallow, and investigate appropriate treatments accordingly. (See Vose et al., 2018, A survey of clinician decision making when identifying swallowing impairments and determining treatment. JSLHR). Also, a clear explanation is needed for why the retrograde flow seen in Case Study #4 is attributed to a Zenker's diverticulum. The patient has GERD in his history, which is more frequently than Zenker's the source of backflow as seen in the study. Was a Zenker's previously identified via videofluoroscopy? This is not clear. Finally, while the use of barium for thin liquid makes sense from the standpoint of visualization, more contrast color (green) with other consistencies would help with demonstrations of bolus flow and residue. It was difficult to distinguish between test material and secretions, but that may just be a function of transferring the video to its current format.
