Excellent information. We all need to be reminded how important our documentation really is. The focus of good, quality documentation has been superseded by the over-stressed demand for point of service documentation. Thank you for this very important reminder!

It is a hard topic to get many people fired up about -- as important as it is. It was a good presentation, but I still want to hear more about about *how* to manage documentation with the type of clinical work I do. I have no issues with it during artic therapy and similar... but complex ASD and AAC cases are often much harder to document because you are doing lots of aided language stimulation and the like and "results" will be unpredictably several sessions down the road. I have my systems, but I keep seeking out PD like this hoping for more examples of how to make it work -- rather than just being reminded about how important it is. We all know the importance of clinical documentation -- but we avoid it at times because it is a very tricky art that we rarely get to see examples of by fellow people.

The course was excellent as it addressed many current , new as well as numerous protocols for documentation in the school based / telehealth speech setting. Many colleagues have varying levels of comprehension, application including relying on multiple and in my opinion unstructured, confusing methods often creating situations where documentation is sub standard , would not clearly describe the skills,and knowledge which make speech therapists necessary individual or part of a team with unique skilled abilities, knowledge and application of such to be deemed necessary to approve/ reimburse for their specialized skilled care from dx- discharge. In a school environment or Independent contract service provider for schools including on-site and telehealth. To my knowledge and experience with numerous school ditricts, states, etc there doesn't seem to be an ICF or common language for school and telehealth-speech service providers. The course was excellent because it directly and effectively explained ICF, value, and the rational for creating and maintaining documentation of what, why how, outcome, and other key ingredients one would expect to see in a hospital, SNF, etc. I hope there is or will soon be a similar common paradigm/ universal means of applying the same standards, goals used as a resource for clinicians who are in settings where ..the entire process can vary from school district to school district resulting in mass confusion and loss of time to effectively provide value based therapy with optimal outcomes.
