The clear information pertaining to Medicaid was so helpful to me. I have always had questions about how thorough my documentation needs to be for individual sessions. Even though there is no clear cut answer across states and/or districts to this question, the presenter's responses encouraged me to continue adding to my documentation process so that I can practice keeping thorough daily data sheets that include the student's performance, goals, IEP minutes, activities completed, number of other students present in the group, anecdotal notes, etc.
